The upside of my misadventure at the dentist’s office is that I’ve always taken extreme precautions to avoid my own patients feeling pain. Often, patients tell me they felt nothing at all when I gave them an injection. I typically use topical aerosol and intradermal anesthetics, and before ultrafine needles were available in Canada, I used to order them from Europe and the United States. Using very fine needles can prevent vasovagal shock—fainting of the patient—which isn’t a pleasant experience for the patient or the doctor. I’ve always taken the time it takes to insert a needle, to the point where I’ve been able to feel when the needle was ready to go deeper without causing the patient any pain. I’ve even suggested to some very fearful patients that they try to visualize the needle entering the skin very gently.

Patients with fibromyalgia may experience an unpleasant feeling with muscle injections, not due to the needle being inserted, but because the injected substance creates pressure in the muscle. In fact, this reaction can be an indicator that a patient may have fibromyalgia, even in the absence of other factors. With all fibromyalgia sufferers, it’s important to inject the substance very slowly to ensure the patient feels no unduly unpleasant pressure, even if the pain for which they consulted will have almost completely disappeared a few minutes later.
